Case Management

Case Management Services, also known as Community Psychiatric Support Treatment (CPST), includes the successful treatment of behavioral health concerns where they occur – at home, school and in the community. Community Support Workers engage with children and their caregivers to manage a child’s day to day behavioral health symptoms. Families are connected to mental health, psychological, psychiatric, social, medical and natural supports. Each family receives a comprehensive plan coordinated with other community resources to achieve their personal goals.

Along with offering CPST to children and families living in the community, youth in both residential and educational programs work closely with a CPST worker.

All Outpatient Mental Health Services

Diagnostic Assessment

No referral needed – ages 3+

Assists in identifying suspected behavioral health and educational concerns, and provide ongoing treatment recommendations.

Medication Management

No referral needed – ages 3–22

Psychiatric assessment and medication management for individuals in need of continued psychiatric support.

Therapy & Counseling

No referral needed – ages 3+

Individual, group, and family therapy for
youth and adults in your home, school, or community setting. Office-based settings also available.

Case Management

No referral needed – ages 3–22

Community-based case management working directly with youth and their families wherever the behavioral concern occurs to help connect with resources within the community.

Transition to Independence

No referral needed – ages 14–22

Intensive community-based (outpatient) services focusing on a successful transition into adulthood.

School & Community Partnerships

Embedded in mental health and behavioral health staff and services to public/private schools or other community-based locations.
