CASE MANAGER-Monfort Heights
Our Case Managers play a vital role in assisting our clients and families in achieving their goals and objectives on the road to recovery and success. You will work with one of the best programs and alongside of highly trained specialists to maximize our client’s independence through the use of personal and community resources. You will assess or assist in the assessment of each client’s psychosocial functioning, strengths, problems and needs. Our Case Managers involve and work cooperatively with family members, significant others, and community partners for a successful outcome for our youth.
Case Managers collaborate with therapists, educational staff, psychiatrists and any other professionals involved in the treatment of our client’s. You will write, coordinate and participate in the development of Individualized Service Plans and actively involve the youth and his/her guardian or other support persons to ensure the recovery efforts.
Our ideal candidate will have Bachelor’s degree in related field from an accredited college and six months SED or special population experience. You must be 21 years of age, have a valid driver’s license and proof of current automobile insurance. You should also have a clean driving record with no more than 5 points with no DUI/OVI violations and you must be able to pass a background check.
