Transition To Independence – TIP

Teens and Young Adults

TIP is a community based program to support teenagers and young adults transitioning into adulthood while managing the additional complexities of mental illness. This intensive program helps bridge the gap between the youth and adult mental health systems.

Utilizing a best practice model, The Transition to Independence Process (TIP) Model , emphasis is placed on supporting completion of high school/vocational programming, seeking and retaining employment, and establishing self-sufficiency.

Community support services help young adults make informed choices about life opportunities and services, ensuring timely access to needed assistance, providing opportunities for self-help activities and coordinating all services to meet their individual goals.
