Digitally Sign to Accept Your Forms
We hope to make this process as easy as possible for our staff and clients to complete your forms.
- Please follow the prompts and complete all fields.
- In gray box, scroll down to green button to begin forms
- Staff to complete all fields with client. Be sure to enter the name of signer: CLIENT or GUARDIAN name and email address as prompted in fields
- Be sure to submit the form after completion by selecting the GREEN SUBMIT button at the end of the form
- Clients should receive an email memo with link to start form after completion
- Be sure the client or guardian checks their email to sign the forms.
